Public Safety Response Report

The tracking of emergency responses to agriculture or farm related fire and/or rescue incidents is unfortunately very haphazard at best. Currently there is no formal reporting mechanism. To help better understand where and how these incidents occur, the following web form was developed to gather information. Please complete as much information as you can. HOWEVER, there should be no personally identifiable information related to the incident (other than information specifically requested.)
